If you would like to participate in our Weekly Lunch Program, please fill out and submit this MDS PSG Weekly Lunch Order Form by Friday, January 5, 2018 at 2:00 pm. This will email your semester order to the MDS PSG. Please make sure ALL required fields (*) are filled out. Then you can either pay the Pizza order online here, (add the order to your Cart and you can pay in full at Checkout), or bring a check made out to "MDS-PSG" to MDS for the total amount listed at the end of this form. All online order forms must be submitted and paid in full either online or by check by Friday, January 5, 2018 at 2:00 pm. Beginning Tuesday, January 9, 2018, and continuing every Tuesday during the semester until Tuesday, May 29, 2018, we order cheese pizza for participating students. This is a 20-week, semester-long fund raising program for the Montessori Day School Parent Support Group and we do not accept weekly orders for pizza. Pizza is $6 for 1 slice = $120.00 Total, $8 for 2 slices = $160.00 Total, and $10 for 3 slices = $200.00 Total. All orders are tax-deductable. If a field trip, special event or inclement weather day falls on a Tuesday, we will notify you of the rescheduled pizza day. Pizza Delivery Dates: (Mark your calendar and don’t pack lunch!)
